Register in two ways:

1.  Download the registration flyer.  Complete the registration flyer and either mail or fax your completed registration form to:

2. Payments may be made by check (payable to VSPE) or by Visa/MasterCard.  A signed registration form will be required if you are paying by Visa/MasterCard.  Prices quoted are for registration prior to May 28, 2004. Registration received after that date, will be subject to an additional $50.00 late registration fee.

Refunds
All cancellations must be received in writing by mail or fax. Full refunds will be made for cancellations received by May 15, 2004. Refund requests made after that time will not be honored, and the participant or sponsoring organization is responsible for the fee. Anyone who is registered but cannot attend may send a substitute.

Registrants must make their own arrangements for accommodations.
